Please note that due to the ongoing health crisis, we have implemented new visitor procedures, including some questions related to your recent health. This information will be shared only as necessary among Jikoji personnel.

In addition to the form below, we are asking that visitors provide confirmation they have completed a course of coronavirus vaccination before arriving at Jikoji. Please attach a photo or a scan confirming vaccination to an email and send it to: vaccineconfirmation@jikoji.org. Here too, this information will be kept private. (If you previously submitted this confirmation ahead of a prior visit, you need not send it again.

CDC GUIDELINES.
For the health and peace of mind of all, Jikoji is continuing to follow basic CDC guidelines. Please be prepared to wear a mask at all times both indoors and outdoors, to maintain social distance of at least 6-feet from others, to wash your hands regularly, and to use hand sanitizer as appropriate. Your host will show you which facilities are available for your use. If you do not have a mask we can provide you with one.

After submitting this form, and also sending us your vaccine confirmation, please be sure to wait to receive confirmation of your visit before arriving at Jikoji.
